Full-Time Community Health Workers, Hackensack Meridian Health (Union, Middlesex, & Northern Monmouth)

Position Summary

Community Health Workers (CHWs) are people who come from the communities they serve. CHWs act as caring neighbors to help people address the social, medical and community problems that lead to poor health. We are looking for CHWs to help our patients deal with the ‘real life’ issues that keep them from staying healthy, help with the tasks of getting medical care, and work on health goals by doing things like planning healthy meals or finding time to exercise.


– Meet with patients and conduct a needs assessment

– Help patients set and achieve their health goals

– Connect patients with community resources such as senior centers, food pantries, and homeless shelters

– Collaborate with a care team to make sure patients’ needs are met

– Help patients with organizing their records, filling their prescriptions, and making follow-up appointments

– Advocate for the needs of people from your community


– Good understanding of the communities and resources of Union, Middlesex, and/or northern Monmouth counties

– Minimum, high school diploma or GED

– Experience in social service, outreach, customer service, or a related field

– Successful completion of paid training upon hire


– Day shift, Monday – Friday

– Schedule includes work in office, community, and telework settings

– Candidate must have reliable transportation. Reimbursement for travel is provided.
