Media Release and Consent

In consideration of my participation in and contributions to the “Souls of Brookdale,” “Student Spotlight,” and or other Brookdale campaigns (the “program/programs”), I hereby authorize Brookdale Community College and its successors, assigns and agents the right to use my name, biography, likeness, voice, performance and visual images (collectively “images”) for distribution via any medium including website, news media, social and email platforms such as Instagram, Facebook, LinkedIn, Constant Contact, and Twitter. I further authorize the Brookdale Community College and its successors, assigns and agents to edit the program, including my performance therein, in whole or in part, in or out of context, and to make recordings or copies of the program in whole or in part, by any other means now known or hereafter devised, for any lawful purpose throughout any medium of distribution throughout the world in perpetuity.

I acknowledge that the Brookdale Community College is the sole owner of all rights to the program/images and any and all recordings thereof. I understand that I am to receive no compensation for my participation therein or the uses thereof. I also understand that Brookdale Community College is not obligated to broadcast or otherwise make use of my appearance, performance or images, and that any use that the Brookdale Community College may make of my appearance, performance or images is at its sole discretion.

Should an outside agency be interested in publishing my contribution(s) beyond the College’s outlets, I hereby consent to Brookdale Community College providing me with the contact information of the agency.
