Position Summary

The Community Health Worker (CHW) serves as a trusted liaison between patients, healthcare providers, and community resources to improve health outcomes and reduce barriers to care.

Job Responsibilities

The CHW supports patients by providing health education, care coordination, resource navigation, and advocacy, with a focus on addressing social determinants of health (SDOH).

The CHW works collaboratively with interdisciplinary teams to promote access to comprehensive, patient-centered, and culturally responsive care consistent with the mission.
